H2 Health Prime Living offers 1:1 patient care in senior living communities with a part-time or PRN schedule. The therapist will evaluate and treat a geriatric caseload, including dysphagia, cognitive impairment, and neurologic disorders, and will implement problem-solving programs.

Independent work and teamwork are essential to meet each patient's functional needs. Experience with outpatient therapy and active CCC-SLP licensure are required, with opportunities for professional growth.

Qualifications

• Education: A graduate of an accredited Speech Language Pathology program.
• Experience: Prior experience in outpatient therapy is a plus. Experience with dysphagia is required.
• Licensing: Active CCC-SLP state license.

Responsabilités

• Evaluate and treat a geriatric caseload 1:1.
• Address dysphagia and cognitive impairment, with problem-solving programs.
• Work independently and in teams, making decisions based on patients' functional needs.
