Position Overview

The Social Worker - Personal Care Home, as part of a multidisciplinary team, provides social work services to residents and families, including counselling, consultation, education, and advocacy. The role involves identifying concerns within the facility and working collaboratively to implement appropriate psycho-social interventions.

The incumbent exercises initiative and judgment in prioritizing work and handling unusual matters, aligning with the mission, vision, and policies of Southern Health-Sant Sud.

Experience
  • At least one (1) year of experience as a Social Worker in a long-term care setting.
  • Experience working within a multidisciplinary team.
Education
  • Graduate of an accredited Faculty of Social Work.
Certification / Licensure / Registration
  • Current registration with the Manitoba College of Social Workers.
Qualifications and Skills
  • Knowledge of Personal Care Home Standards regulation.
  • Ability to respect and promote cultural diversity.
  • Proficiency in both official languages is essential for bilingual positions.
  • Ability to maintain confidentiality across various mediums.
  • Good attendance and work record.
  • Valid Class 5 driver’s license, insured vehicle, and liability insurance of at least $1,000,000.
Physical Requirements
  • Ability to meet physical and mental demands of the role.
  • No hazardous or significantly unpleasant conditions.
  • Occasional evening and weekend work may be required.
  • Travel to regional facilities may be necessary.

This term position may end earlier as outlined in your collective agreement.
