The Social Worker - Personal Care Home, as part of a multidisciplinary team, provides social work services to residents and families, including counselling, consultation, education, and advocacy. The role involves identifying concerns within the facility and working collaboratively to implement appropriate psycho-social interventions.
The incumbent exercises initiative and judgment in prioritizing work and handling unusual matters, aligning with the mission, vision, and policies of Southern Health-Sant Sud.
This term position may end earlier as outlined in your collective agreement.
