Zusammenfassung

Derbyshire County Council is seeking a Social Worker to join the High Peak South Adult Social Work Team. You will undertake Care Act 2014 assessments, apply the Mental Capacity Act in practice, and develop person-centred, strengths-based support plans.

You will work collaboratively with individuals, families and partner agencies to safeguard and support adults 18+ with long-term needs, including transitions from adolescence. A DBS check is required.

Qualifikationen

• Knowledge of Care Act 2014 and Mental Capacity Act 2005.
• Experience in adult social care and safeguarding (preferred).
• Ability to work flexibly and collaboratively.

Support

Regular supervision and peer support, You will work within a generic adult social care team supporting:

• Adults aged 18+ with long-term care and support needs
• Young people (16+) transitioning into adulthood, particularly those with learning disabilities

We are seeking a motivated and skilled Social Worker to join our well-established High Peak South Adult Social Work Team. This is an excellent opportunity to become part of a supportive, long-standing team including a Team Manager, Senior Practitioner, Social Workers and Social Care Practitioners., Knowledge of Care Act 2014 and Mental Capacity Act 2005

This role requires a Disclosure and Barring Service (DBS) check due to work with vulnerable adults. Criminal records will be considered on an individual basis.
