Derbyshire County Council is seeking a Social Worker to join the High Peak South Adult Social Work Team. You will undertake Care Act 2014 assessments, apply the Mental Capacity Act in practice, and develop person-centred, strengths-based support plans.
You will work collaboratively with individuals, families and partner agencies to safeguard and support adults 18+ with long-term needs, including transitions from adolescence. A DBS check is required.
Adults 18+ with long-term needs + learning disability transition cases from age 16
Care Act 2014 assessments + Mental Capacity Act (MCA) work + Safeguarding + Strengths-based support planning
Mix of physical disabilities, learning disabilities, and transition-to-adulthood work
Regular supervision and peer support, You will work within a generic adult social care team supporting:
