Buckinghamshire NHS Trust is seeking a RRIC Community Healthcare Assistant Practitioner to join the Wycombe Rapid Response and Intermediate Care teams. You will manage a caseload of patients with acute and chronic conditions, working with physiotherapists, nurses, and paramedics to maximise independence.
You will deliver clinical and therapeutic care within set protocols, assess patients, and plan/implement treatment. This role supports discharge planning and quality care.

Our Wycombe Rapid Response and Intermediate Care teams are busy places to work and you will be using your skills to support a caseload of patients with both acute and chronic conditions enabling them to maximise their independence. The role has been developed to support the clinical case managers and you will be working alongside Physiotherapists, Occupational Therapists, Nurses and Paramedics as well as rehabilitation focused healthcare assistants. You will carry your own caseload as well as supporting and reviewing people receiving services from the team.
