The Kansas Statewide Farmworker Health Program (KSFHP) is looking for a Program Consultant to join their team of care coordinators to connect clients to healthcare. Care Coordination emphasizes preventative care for chronic conditions, including diabetes, hypertension and behavioral health and improved access to healthcare for vulnerable farmworkers.
This is a fully remote position; however, candidates must reside in Kansas or be willing to relocate. The role specifically serves the Eastern region of Kansas, and applicants should live within this area. Travel within the region, approximately 25%, is required. Ability to communicate and have attention to detail are necessary skills for this position. Bilingual in Spanish/English encouraged to apply.
Case Management Activities: Identify migrant and seasonal farmworker clients eligible for primary health care and other services within the assigned geographical area of the state. Up to 25% travel within assigned region. Assess client need and make referrals to appropriate agency or provider. Work with clients and providers to assure adequacy and continuity of care. Coordinate, track, and conduct follow-up activities to support the client's ability to follow through with treatment plans. Maintain essential case management and health care encounter records. Coordinate the work of Health Promoters in the region to ensure health education and continuity of care. Provide health education and resources to clients. Comply with standards of client and administrative confidentiality. Maintain security of confidential records.
Job Responsibilities may include but are not limited to the following:
Voucher Program Operation: Record information in automated databases for individual clients, families, providers, access point agencies using computerized information system of the Statewide Farmworker Health Program. Submit reimbursement vouchers and billing claims to data entry clerk weekly.
Provider Network: Identify available resources within the state, region, or community. Negotiate contracts for care and maintain working relationships with a network of access point agencies and private providers. Provide technical assistance, consultation, and culturally appropriate health information to improve the delivery of services to migrant and seasonal farmworkers and their families. Strengthen and initiate partnerships with local and state agencies in the area. Coordinate care with state tuberculosis program staff and other state and local program officials to assure compliance with program and treatment regimes. Obtain consultation from state epidemiologist, primary care provider, or other health personnel, as needed, to develop a plan of care for clients with complex conditions.

Education:
High School, GED
Licensing & Certification:
Valid Driver's License - Incumbent is required to have and maintain a valid driver's license when operating a state vehicle, a private vehicle, or a rental vehicle for the benefit of the State.
Minimum Qualifications:
Bachelor's degree in social science, nursing, sociology, psychology, social work or other health related field
Three years of analytical data work or relevant program related work. Bachelor's degree in social or health services may be substituted for experience as determined relevant by the agency.

General knowledge in one of the following areas: primary healthcare, nursing, social work or behavior
Ability to maintain good working relationships with local agencies
