Provides psychosocial services to patients treated by the facility, including in-center and home dialysis patients (if applicable), utilizing Social Work Theory of Human Behavior and accepted methods of social work practice. Works with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for patients. Collaborates with the interdisciplinary team to inform, educate, and support staff in understanding the emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families to ensure comprehensive quality care. Supports the FMCNA commitment to Quality Indicators and Outcomes, and actively participates in process improvement activities to help patients achieve FMCNA Quality Goals.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Patient Assessment / Care Planning / Counseling: Assesses patients' psychosocial status, strengths, and areas of need affecting rehabilitation and treatment outcomes. Participates in care planning with the patient and healthcare team to identify interventions that meet treatment goals and improve quality of life. Utilizes patient education programs, social work theories, and quality of life measurement tools. Provides counseling services to help patients adjust to dialysis and seek transplant options. Facilitates referrals to community resources and assists with advance directives and DNR discussions, ensuring patient understanding and informed decision-making. Documents interactions and interventions per policies.
  • Quality: Provides psychosocial support and interventions addressing non-adherence and quality of life concerns. Participates in monthly quality review meetings and reports on relevant quality indicators.
  • Patient Education: Assesses patient knowledge and barriers related to kidney disease and treatment adherence. Provides tailored education, facilitates transplant referrals, and educates about psychosocial issues and patient rights. Ensures understanding of policies and rights.
  • Patient Admission and Continuity of Care: Reviews patient rights, grievance procedures, and addresses immediate needs. Supports scheduling and discharge processes, identifying root causes for discharge requests and sharing concerns with leadership.
  • Insurance and Financial Assistance: Acts as liaison with insurance coordinators, providing information and education on payment options and changes affecting coverage. Assists with billing questions and issues.
  • Staff Related: Assists in staff hiring processes, maintains updated resource lists, provides training on psychosocial topics, and participates in team huddles. Discusses urgent patient issues with staff.
  • Adheres to caseload guidelines and performs other duties as assigned.

EDUCATION AND CREDENTIALS: Masters in Social Work, valid state license required.

EXPERIENCE AND SKILLS: 2-5 years of related experience preferred.
