Zusammenfassung

Jobtailor is seeking a remote Health Engagement Specialist to support patient adherence and engagement. You will review patient video submissions, coach patients via video and messaging, and collaborate with nursing staff to address barriers and safety concerns.

Ideal candidates have experience in digital health, remote monitoring, and Medicaid experience, and can work independently from Puerto Rico or other U.S. jurisdictions. Bilingual Spanish is preferred.

Qualifikationen

• 3-5 years of experience in healthcare engagement, care coordination, adherence support, or a similar role
• Experience interacting directly with patients in healthcare settings
• Strong written and verbal communication skills
• Exceptional attention to detail
• Comfort reviewing patient-submitted video content
• Ability to work independently in a remote environment
• Proficiency with CRM systems and mobile applications
• Strong time management skills in a queue-based workflow
• Candidates must reside in Puerto Rico and maintain Puerto Rico residency throughout employment
• Must be legally authorized to work in Puerto Rico/United States without employer sponsorship
• Visa sponsorship is not available
• Preferred: background as a medical assistant, pharmacy technician, community health worker, certified health coach, or directly observed therapy worker
• Preferred: experience in digital health or remote patient monitoring
• Preferred: experience working with Medicaid or underserved populations

Aufgaben

• Review assigned asynchronous patient medication videos within established turnaround times and document adherence accurately
• Confirm proper oral medication ingestion and identify adherence barriers
• Deliver personalized video feedback and coaching to reinforce medication-taking behaviors
• Use video messaging, phone, and SMS to support patient engagement and adherence
• Conduct proactive outreach to patients who miss submissions or show declining adherence
• Support re-engagement of patients at risk of attrition
• Serve as frontline support for the RN team and connect patients with appropriate services
• Reinforce care plans, medication schedules, appointment adherence, and physician visits
• Screen and document social determinants of health
• Provide approved community and program resources
• Escalate medication questions, side effects, safety concerns, health issues, and other risks to nursing staff
• Assist with basic troubleshooting of the mobile application and adherence platform
• Maintain accurate, compliant documentation
• Meet quality, compliance, and video engagement performance standards
• Participate in quality audits, team meetings, and ongoing professional development
• Provide feedback to improve video engagement strategies, patient experience, and adherence workflows
