Job summary

NHS Lanarkshire is seeking a skilled clinical pharmacist to provide a safe, patient-centred specialist pharmaceutical care service to a defined patient group. You will assess needs, develop and action care plans, and contribute to medicines governance and quality improvement.

You will communicate clearly, work within a multidisciplinary team, engage in CPD, and support service delivery in the specialty area. A GPhC registration and independent prescribing (or progress toward) are required.

Qualifications

• Masters of Pharmacy Degree.
• Registered member of General Pharmaceutical Council.
• Independent prescribing qualification or working towards.
• Postgraduate training and CPD in clinical pharmacy practice.
• Post-registration experience in clinical pharmacy.

Responsibilities

• Deliver patient-centred specialist pharmaceutical care to patients in specified clinical area(s).
• Communicate effectively, placing the person at the centre of interactions.
• Assess pharmaceutical care needs and develop/action a care plan.
• Identify training needs and participate in CPD activities.
• Promote pharmacy services and develop the profession.
• Undertake and participate in research or audits within the area of practice.
