A health care organization is seeking a Care Coordinator in Ontario to assess patient needs, coordinate care delivery, and support families. This full-time, temporary role offers an hourly wage of $41.29 to $51.13, and requires membership with the College of Nurses of Ontario and bilingual proficiency in English and French.

Care Coordinator (Full‑Time, Temporary from December 1, 2025 to February 5, 2027; Location: Haileybury)
You will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health‑care practitioners. Whether you work in our office, in the community, or a health‑care facility, you will play a lead role in providing connected, accessible, patient‑centered care and be supported by our collaborative team that includes over 9,000 regulated health‑care and other professionals.
French Proficiency: French required
Salary: Hourly wage of $41.29 to $51.13.
Terms of Employment: Temporary, Full‑Time.
