Delta Division of Family Practice Lead the Transformation of Primary Care in South Delta
Are you a strategic and collaborative health care leader with a passion for enhancing community-based primary care? If so, this exciting opportunity to serve as the Primary Care Network (PCN) [Director/Manager] in South Delta may be your next career move.
Reporting to the Executive Director, you will be responsible for operationalizing the Ministry of Health's PCN initiative within the region. Working closely with the local Division of Family Practice, health authority, First Nations communities, and other key stakeholders, you will oversee the day-to-day implementation of the PCN Service Plan.
As the PCN [Director/Manager], you will leverage your strong project management, communication, and facilitation skills to drive collaborative relationships, foster community engagement, and champion the principles of primary health care, population health, and health promotion. With a proven track record in integrated service delivery and a deep understanding of the PCN model, you will lead the transformation of primary care in South Delta.
To succeed in this pivotal role, you will need a minimum of 5 years' experience in a distributed leadership capacity within the health sector, as well as expertise in quality improvement, change management, and cultural safety. A post-secondary degree in a health-related discipline is required, with a master's degree preferred.
