Job summary

CarePartners in Mississauga is seeking a Team Coordinator to ensure timely scheduling of PSWs, nurses, and therapists and to manage communications between patients, families, and staff. The role requires organizing referrals, coordinating care, and handling routine inquiries with a customer service mindset.

The position is full-time with defined shifts, including weekends. Training is provided, and a competitive compensation grid is offered, with an hourly range of $19.67 to $26.40 and

Qualifications

• Post-secondary education preferred.
• 1 year of admin or customer service experience required.
• Strong interpersonal and customer service skills.
• Ability to work independently and as part of a team.
• Excellent verbal and written communication skills.
• Experience in health care is an asset.

Responsibilities

• Serve as the bridge between patients, families, and CarePartners staff.
• Receive routine referrals and intake information from funders and clients.
• Develop and coordinate service schedules considering relevant factors.
• Inform workers of new assignments and changes in schedules.
• Enter information into the automated information system/database.
• Generate schedules and reports for the Supervisor.
• Handle inbound calls and emails related to service.
• Perform other administrative duties as required.
