Job summary

Stonehenge Therapeutic Community in Ontario seeks a Support Worker for the Crisis Stabilization program. This eight-bed residential unit provides de-escalation and stabilization for individuals in crisis related to mental health and substance use, with links to long-term community supports.

You will work as part of an interdisciplinary team delivering practical and emotional supports, including transportation on occasion, with evening/weekend rotations and holiday coverage.

Qualifications

• Completion of a College Diploma or University Degree relevant to mental health, addiction, or social services.
• Minimum one year of progressive experience in addictions or mental health; trauma-informed practice an asset.
• Experience with individuals experiencing mental health and/or substance use-related crisis.
• Experience in social service settings with knowledge of case management systems.
• Lived experience related to addictions and/or mental health an asset.
• CSC Clearance conducted by PWGS Canada.
• Standard First Aid/CPR Level C.
• Reliable vehicle and Ontario driver's license (G2 or G).

Responsibilities

• Support participants in achieving wellness goals related to substance use and mental health.
• Develop and implement crisis intervention and risk management plans.
• Support life skills development aligned with client goals.
• Assist with referrals to internal and external programs and services.
• Provide service navigation and appointment accompaniment as needed.
• Coordinate services with longer-term supports and discharge planning.
• Facilitate psycho-education and support groups.
• Engage in conflict resolution and maintain confidentiality and documentation standards.
• Assist with on-call duties evenings and weekends on a rotation.

About the Role:

The Crisis Stabilization program is an eight-bed residential crisis service, providing an alternative to hospitalization or criminal detention for individuals experiencing a crisis related to their substance use and mental health. Staff provide de-escalation and stabilization of the immediate crisis and connection to the services needed to enable safe and stable community living that addresses client health and wellbeing longer term.

The Support Worker will work as a member of an interdisciplinary team providing a variety of practical and emotional supports to facilitate crisis stabilization and effective community re-integration. Transportation of clients may be required; Evening/weekend work is required on a rotational basis.

Hours of Work:

• Days (7:30am-5:30pm)
• Hours are scheduled on a 2-week rotation and includes working every second weekend. Work on statutory holidays is required where the day falls on the regular working day.
• On call work is required on a rotational basis, in addition to regular schedules (overnight staff excluded).

Qualifications:

• Standard First Aid/CPR Level C
• Reliable Vehicle, Ontario Driver's license (G2 or G)
• Must provide proof from Insurance provider of 1) at least $2 million in general liability and 2) have declared scope of vehicle use to Insurance provider (overnight staff excluded)
• Willing to transport clients to appointments in your own vehicle (overnights excluded)
• Travel within the Wellington Guelph area

Role Overview:

Client Support

• Support participants in achieving the wellness goals they have identified in relation to substance use, mental health, and other domains of life
• Support plans for crisis intervention and risk management
• Support life skills development as identified in the clients personal goals
• Assist with making referrals to internal and / or external programs or services, as appropriate
• Assist participants to connect to local resources by providing service navigation support and appointment accompaniment when necessary
• Support the coordination of services with longer term supports, as appropriate
• Support comprehensive and collaborative discharge planning
• Facilitate psycho-education and support groups for participants in the house
• Engage in conflict resolution as necessary
• Maintain appropriate confidentiality and documentation standards as required by the Agency

Program Support

• Monitor the Crisis Stabilization environment (building)
• Conduct bed checks
• Provide medication to clients and complete required documentation
• Monitor withdrawal symptoms as necessary using withdrawal monitoring scales
• Participate in program development as neededSome light cooking and housekeeping responsibilities required, including room inspections and turnover upon client discharge, laundry, and sanitization of common spaces
• Provide education to community partners as required
• On a rotational basis, provide on-call services during evenings and weekends (overnights excluded)

Administration and Reporting

• Involvement in program evaluation, data collection/input into CaseWORKS
