Job summary

Stonehenge is seeking an Intensive Case Manager for its Crisis Stabilization program, a residential eight-bed service addressing crises related to substance use and mental health. You will work within an interdisciplinary team delivering intake, case management, and care coordination, supporting clients toward safe community living.

Evening/weekend rotations and on-call duties are required. The role requires a college diploma or university degree in mental health, addiction, or social services,

Qualifications

• Completion of a College Diploma or University Degree relevant to mental health, addiction, or social services.
• Minimum one year of progressive experience in addictions or mental health; trauma-informed practice an asset
• Experience working with individuals experiencing mental health and/or substance use crisis
• Experience in social service settings with working knowledge of case management systems and planning techniques
• Lived experience related to addictions and/or mental health an asset
• CSC Clearance conducted by PWGS Canada
• Standard First Aid/CPR Level C
• Reliable vehicle and Ontario Driver's license (G2 or G)
• Must provide proof of at least $2 million general liability and declared vehicle use scope to insurer
• Willing to transport clients to appointments in own vehicle (overnight excluded)

Responsibilities

• Provide intake, case management and care coordination supports to Crisis Stabilization clients as part of an interdisciplinary team.
• Contribute to a 24-7 service model; participate in on-call rotations as needed.
• Coordinate transportation for clients when required.
• Assist with plan development and service linkage to support safe community living.
• Participate in rotating evening/weekend work as part of the schedule.

Job description

About the Role:

The Crisis Stabilization program is an eight bed residential crisis service, providing an alternative to hospitalization or criminal detention for individuals experiencing a crisis related to their substance use and mental health. Staff provide de-escalation and stabilization of the immediate crisis and connection to the services needed to enable safe and stable community living that addresses client health and wellbeing longer term.

The Intensive Case Manager will work as a member of an interdisciplinary team to provide intake, case management and care coordination supports to Crisis Stabilization clients and as part of a 24-7 service. Transportation of clients may be required; Evening/weekend work is required on a rotational basis.

Hours of Work:

Full time days (7:30am-5:30pm). Hours are scheduled on a 2-week rotation and include working every second weekend. Work on statutory holidays is required where the day falls on the regular working day.

On call work is required on a rotational basis, in addition to regular schedules (overnight staff excluded)
