Job summary

March-of-Dimes-Canad is seeking a Full-Time Senior Brain Injury Rehabilitation Worker in the New Liskeard area, ON. The role involves delivering community support services to adults with brain injuries, following a structured plan with the Regional Service Coordinator (RSC).

You will manage a caseload of 9 clients, complete required reports, and collaborate with multidisciplinary teams to meet individual needs. Travel within the region is required.

Qualifications

• 2–5 years experience in social services/health sector coordinating brain injury services.
• Experience coordinating services for individuals with brain injury is an asset.
• Community-based experience with brain injury or related disabilities.
• Strong organizational and time-management skills.
• Problem solving and conflict resolution skills.

Responsibilities

• Implements programs designed to enhance efficiency and safe delivery of Community Support Services for brain injury clients with RSC support.
• Manage a caseload of 9 clients with one office block per week.
• Complete daily report writing, incident reports, IRP, and quarterly reporting with RSC.
• Balance caseload flexibility with other duties as assigned; collaborate with multidisciplinary teams.
• Provide long-term support & service coordination to enable adults with brain injuries to remain in the community.
• Offer cognitive and compensatory strategies to support functional goal setting.
• Travel within region on a daily basis.

The opportunity

We are currently looking to fill one new opportunity for a Full-Time Senior Brain Injury Rehabilitation Worker position in the New Liskeard, ON Region

• Schedule: Monday to Friday
• Starting wage: $27.01 per hour. This compensation is inclusive of a $3.00 per hour wage enhancement from the Ontario government for hours spent directly supporting clients. The wage enhancement does not apply to training and paid leave.
• Hours: 30 hours per week

Responsibilities:

• Implements programs designed to enhance the efficiency and safe delivery of Community Support Services to clients with Brain Injury with the support of the RSC
• Manage a case load of 9 clients with one office block per week.
• Working collaboratively with the RSC in completing all daily report writing, incident reports, Individual Rehabilitation Plan, and quarterly reporting
• Ability to manage a caseload with flexibility while responding to other duties as assigned.
• Work collaboratively with the multi-disciplinary teams to ensure individual needs are met through functional goal setting.
• Act as a resource to staff and families in the provision of long-term support and service coordination for complex or catastrophic cases to enable adults with Brain Injuries to remain in community.
• To provide cognitive and compensatory strategies that support functional goal setting
• Travel within region on a daily basis

Qualifications and experience:

• 2 – 5 years experience in the Social Services/Health Sector, coordinating services for individuals with Brain Injury is an asset
• 2 Years of Post Secondary Education

• Concrete knowledge of the effects of an ABI is an asset, training will be provide
• Community based experience working with individuals with a Brain Injury, physical disabilities or special needs
• Highly organized and excellent time management skills
• Problem solving and conflict resolution skills
• Knowledge of behaviour management principles
• General understanding of computer hardware and software
• Strong ability to assess and determine effective compensatory strategies for individuals with brain injuries.
• Valid Licence and access to a car
• Insurance liability of $1,000,000.00
