Job summary

March of Dimes Canada is hiring a Rehabilitation Services Coordinator in Simcoe County for a full-time, 12-month contract. You will create and track progress through an Individual Rehabilitation Plan, meet funder targets, and collaborate with Behaviour Consultants to deliver in-home and community services.

Daily travel within Simcoe County is required with mileage reimbursement. The role requires post-secondary education in Human and Social Services and at least three years’ experience in Human

Qualifications

  • Post-secondary education in Human and Social Services is preferred.
  • Minimum of three years’ experience in Human Services field and/or working with people with brain injuries.
  • Experience with report writing is an asset.
  • Current CPR/First Aid is an asset.
  • Experience with group facilitation is an asset.
  • Proficiency with Microsoft Office applications.
  • Strong interpersonal communication and organizational skills.
  • Experience supervising or leading teams.
  • Sound behavioral management and cognitive problem-solving skills.
  • English reading/writing/speaking ability.

Responsibilities

  • Create, implement and track progress through an Individual Rehabilitation Plan according to client’s goals.
  • Meet service deliverable targets set by funders and support duties as assigned.
  • Collaborate with Behaviour Consultants as required.
  • Complete RAI CHA assessments/training provided.
  • Develop and coordinate home-and-community-based services.
  • Maintain reporting (incident, behavioral data, progress)
  • Liaise with social services and health care providers.
  • Schedule hours and supervise a team of rehabilitation workers.
  • Communicate with client, family and team members.
  • Assess client skills and development needs; conduct follow-ups.
  • Participate in required training.

Whether their disability is physical, the result of brain injury, or aging, our more than 2,000 team members assist clients in their homes and across communities, to live as independently as possible.

The Opportunity:

We are currently looking to fill one new opportunity for a Full-Time (12-month contract) Rehabilitation Services Coordinator position in Simcoe County.

  • Schedule: Monday to Friday 9am-4:30pm.
  • Starting wage: $28.43 per hour.
  • Hours: 35 hours per week.
Responsibilities:
  • Create, implement and track progress through an Individual Rehabilitation Plan according to client’s goals
  • In conjunction with the community support supervisor, meet service deliverable targets set forward by our funders as well as support with duties as assigned
  • Work collaboratively with Behaviour Consultants where required
  • Complete RAI CHA assessments/training provided
  • Develop, coordinate and implement the delivery of specific services within the client’s home and various settings within the community
  • Reporting responsibilities (incident reports, behavioral data collection, verbal reports and progress reports)
  • Liaise with other social services agencies and health care providers
  • Responsible for the scheduling of your hours as well as a team of rehabilitation workers that will report to this position
  • Communicate with client, family, and team members
  • Assess client’s relevant skill strengths and development needs and conduct follow-up assessments
  • Participate in any required training within the requested time frame
  • Daily travel within Simcoe County - Time for Travel and Mileage reimbursement
Qualifications:
  • Post-secondary preferred in Human and Social Services
  • Minimum of three years’ experience in Human Services field and/or working with people with brain injuries.
  • Experience with report writing an asset
  • Current CPR/First Aid an asset
  • Experience with group facilitation is an asset
  • Experience with Microsoft Office applications
  • Proven consistency in work performance, including teamwork, reliability and flexibility
  • Excellent interpersonal communication and organizational skills
  • Demonstrated experience supervising or leading teams
  • Sound behavioral management skills, cognitive problem-solving skills.
  • Working knowledge of brain injury and cognitive rehabilitation is an asset
  • Ability to speak, read, and write english
