Our integrated teams provide assertive case management to clients experiencing multi-barriers. It is primarily an outreach position in the Downtown Eastside of Vancouver requiring travel on foot and by car through the community and in the Vancouver Region. Much of the work takes place in the community and includes home visits at Downtown Eastside hotels, attendance at community resources such as shelters, mental health drop-in centres, and addiction focused resources.
In this role, you will assess, supervise and support adults under community supervision, helping them address their needs, make positive changes and work toward successful outcomes. You will prepare information to support court decisions, develop individualized case management plans and connect clients with appropriate community resources and interventions. Working independently and collaboratively with colleagues and community partners, you will help promote public safety, accountability and fair, respectful support that recognizes each person's circumstances and potential for change.
