We are hiring dynamic, passionate Speech Language Pathologists (SLPs) who want to be part of the transformational change in home and community care by providing care in a truly client-centered approach. You will be part of a diverse care team that will provide services to a roster of clients in the community. You will conduct joint care planning with a focus on client-centered goals addressing clients’ physical, cognitive, social, emotional, and spiritual domains, creating a holistic care plan. Each client is assigned a primary care clinician, a member of the interdisciplinary team who will lead care planning for that client and facilitate the interdisciplinary team to reach client’s care goals. As part of the SE Transitions team, you will be accountable for the care and services you provide and the outcomes your clients achieve.

Responsibilities

  1. Conducting assessments

Requirements

  1. A graduate of an accredited program in Speech Language Pathology and a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
  2. Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred
  3. Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high-performance interdisciplinary rehabilitation team
  4. Sound knowledge of community resources is an asset
  5. Effective interpersonal, organizational and communication skills
  6. A positive work and attendance record is essential
  7. Ability to adapt to new computer applications

This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community-based rehabilitation setting.
