Job summary

The Town of Strathmore is seeking a passionate and community-minded Special Projects Coordinator, Link Worker to support the Senior Social Prescribing program, connecting older adults with community resources and supports to enhance health and wellbeing.

Reporting to the Manager, Community & Social Development, you will build partnerships, coordinate programs, and contribute to a thriving, inclusive community in Strathmore.

Qualifications

  • Post-secondary education in a social service-related field is required.
  • Experience working for a non-profit organization is preferred.
  • Knowledge of issues affecting older adults and community resources is beneficial.

Responsibilities

  • Develop and implement the Senior Social Prescribing program.
  • Support connection of older adults to community resources and supports.
  • Collaborate with the Family and Community Support Services team and report to management.

The Town of Strathmore is seeking a passionate and community-minded Special Projects Coordinator, Link Worker to support the development and implementation of our Senior Social Prescribing Program. This role will help connect older adult residents with community resources, programs, and supports that enhance their health, well-being, and social connectedness.

What You’ll Do:

Reporting to the Manager, Community & Social Development, the Special Projects Coordinator, Link Worker position will play an integral role in developing and implementing the Senior Social Prescribing program, which aims to build community support to meet the nonmedical needs of our Older Adult residents within our community. The position will also collaborate with and support the Family and Community Support Services team.

Successful applicants will demonstrate on their resume:

  • Post-secondary education in a social service-related field
  • A minimum of two (2) years experience working for a non-profit organization
  • Experience building collaborative partnerships with community organizations or service providers
  • Experience working with older adults is beneficial
  • Knowledge of issues impacting older adults
  • Knowledge of community resources available to support older adults

Or interview questions are designed to assess the following competencies:

  • Excellent communication and interpersonal skills with the ability to easily work closely with older adults, colleagues and community social service providers
  • Ability to develop and foster relationships with diverse social service providers, including older adult residents and other related social service providers
  • Ability to multi-task, adhere to deadlines, and meet project milestones
  • Flexible schedule and availability to accommodate client meetings, social prescribing, programming, and older adult in house meetings or support that may be outside of work hours
