Job summary

Shawenim-Abinoojii is seeking a Southern Care Case Manager to provide holistic case management for South Care homes. The role reports to the Southern Care Program Supervisor and involves regular visits, coordination with guardian agencies, and documentation across licensing processes.

Responsibilities include intake planning, ongoing care planning updates, and collaboration with guardians to support children’s needs.

Qualifications

  • BSW degree or equivalent (or similar education/experience).
  • Excellent conflict resolution skills.
  • Excellent communication, organization, problem solving and team skills.

Responsibilities

  • Facilitate pre-service orientation for new Care Providers.
  • Establish supportive relationships with Care Providers, youth and the support team.
  • Conduct intake care planning meetings with guardian agency staff and Care Providers.
  • Complete ongoing care home documentation and licensing paperwork (Home Studies, Foster Home Licenses, Annual Reviews, Addendums).
  • Maintain awareness of services in each home and communicate with the Program Supervisor and guardian agency.
  • Collaborate with guardian workers to deliver care plans focusing on the child’s holistic needs.
  • Review monthly reports and incident reports to identify challenges and implement solutions.
  • Travel to Winnipeg and surrounding areas as required.
  • Lift and carry items up to 50 lbs.

OVERVIEW

Reportingto the Southern Care Program Supervisor, the South Care Case Manager will beresponsible for providing support to the South Care homes. The Southern CareCase Manager will work within a team that serves to provide comprehensive casemanagement services within a holistic circle of care.

KEYRESPONSIBILITIES
  • Facilitate pre-service orientation for new CareProviders.
  • Establish a positive and supportive relationshipwith Care Providers, youth in the homes, and the support team through being aregular visitor to the home and maintaining ongoing communication by phone andvia email.
  • Conduct intake care planning meetings with guardianagency staff and Care Providers and conduct regularly scheduled care planningupdate meetings.
  • Based on your ongoing awareness of and connectionto the homes, complete ongoing care home documentation and assess and completerequired licensing documentation:
  • HomeStudies and Foster Home License Applications, as needed.
  • AnnualReviews
  • Addendums,as needed.
  • FosterCare Licensing Closures, as needed.
  • Maintain an accurate understanding of the servicesprovided in each home and effectively communicate with the Program Supervisorand guardian agency.
  • Collaborate with guardian worker in delivering careplans for each child and/or family, while remaining focused on child’swholistic needs.
  • Collect and review monthly reports and incidentreports for each home, identify challenges and strategize with the home andcare teams to implement solutions.
  • Liaise with Care Providers to ensure information iseffectively communicated.
  • Communicate to Supervisor the observed trainingneeds for House Parents, Support & Respite Workers, and find those trainingopportunities.
  • Contribute to organization committees based on yourpersonal and professional experience and goals.
  • Regular travel is required for this position(Winnipeg and surrounding areas) – travel and accommodation provided.
  • Ability to lift and carryitems to a maximum of 50lbs
QUALIFICATIONS
  • BSW degree or equivalent(similar education and experience working in the field will be considered)
  • Knowledge in the areas ofthe impacts of colonization and intergenerational trauma
  • Excellent conflictresolution skills
  • Knowledge of the Child andFamily Services (CFS) sector and legislation
  • Good understanding offoster care standards and related licensing paperwork
  • Demonstrate knowledgeand/or understanding of Indigenous perspectives, First Nations culture andtraditional philosophies.
  • Experience developing andsupporting a strong, respectful team-based approach to service delivery.
  • Experience working withfamilies and navigating complex situations.
  • Must have a clear ChildAbuse & Adult Abuse Registry Check, Criminal Record check (withVulnerable Sector).
  • Strong proficiency inMicrosoft Office, word processing (MS Word), Spreadsheets (Excel), E-mail(Outlook) and Internet.
  • Excellent communication,organization, problem solving and team skills.
  • Ability to speakAnishinaabe is an asset.
  • Must have a valid Manitobadriver’s license and a reliable vehicle.
