Social Worker
Location: Markham, Stouffville, Unionville

As a Social Worker, you will assess, resolve, prevent or lessen the impact of psychosocial,physicaland mental health related issues for clients and their families. Care will take place within a residential or institutional setting with referrals generated by a variety of sources including government agencies, employers, insurance companies or private payees. Client groups receiving treatment may includepediatrics for in home or school services and/oradults with medical/health concerns.

- Conduct client assessments related to financial, accommodation, legal or mental healthbarriers
- Assist clients in setting S.M.A.R.T.goals
- Design and implement treatment plan for clients usingevidence basedinformation, best practice, and clinical expertise
- Work collaboratively with a multi-disciplinary team consisting of government agencies, healthcare workers, and other communityresources
- Advocate on behalf of clients and families dealing with mental health or disabilities, chronic medical conditions, addictions, palliative and othersituations
- Identify and link clients and families to available resources within thecommunity
- Offer health teaching to clients, families and caregivers as it relates to client’s medicalcondition
- Evaluate the effectiveness of interventions, client response, and progress toward goal achievement or for need to altergoals
- Communicate with the Health Care team regarding findings, treatments andprogress
- Maintain professional client records related to client encounters and plans in compliance with ParaMed’s policy and the standards applied by the regulatory body or college.
- Maintain the knowledge, skills and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope ofpractice
- Degree in Social Work combined with registration /licensing with provincial regulatory college
- Minimum of 2 years’ experience as a practicing Social Worker preferably in a community setting
- Working knowledge of applicable legislation including Mental Health Act, Substitute Decisions Act, Healthcare ConsentActand others
- Superior oral and written communication skills to effectively interact with clients and families possessing varying abilities forunderstanding
- Effective skills in critical thinking and problemsolving
- Knowledge of applicable community resources available for clients and families
- Current certification (within the past 12 months) of First Aid, CPR completed withinCanada
- Able to provide a current, clear Vulnerable Position Screening (VPS)
