Job summary

ParaMed is seeking a Social Worker to assess and address psychosocial, health and related needs for clients and families in Markham, Stouffville, and Unionville. You will work in residential or institutional settings with referrals from varied sources, delivering evidence‑based plans and advocating for clients.

You will collaborate with a multidisciplinary team, link clients to community resources, and maintain compliant records while staying current with practice standards and legislation.

Qualifications

  • Requires degree in Social Work and registration/licensing with provincial regulatory college.
  • Excellent oral and written communication skills to interact with clients and families.
  • Knowledge of mental health laws and community resources is advantageous.

Responsibilities

  • Assess clients to identify psychosocial, financial, legal or mental health barriers.
  • Design and implement evidence‑based treatment plans with clinical teammates.
  • Collaborate with a multi‑disciplinary team and community resources.
  • Advocate for clients and families facing mental health challenges or disabilities.
  • Link clients to community resources and provide health teaching to clients and caregivers.
  • Maintain accurate client records in compliance with ParaMed policy and regulatory standards.

Job DescriptionSocial WorkerLocation: Markham, Stouffville, UnionvilleOpen for Full-Time WorkAs a Social Worker, you will assess, resolve, prevent or lessen the impact of psychosocial, physical and mental health related issues for clients and their families. Care will take place within a residential or institutional setting with referrals generated by a variety of sources including government agencies, employers, insurance companies or private payees.
