Job summary

ParaMed Home Health Care is seeking a Care and Service Manager (CSM) to lead our clinical supervisors and frontline staff in delivering high-quality patient care across our home health network.

You will oversee care delivery, mentor teams, and ensure compliance with relevant regulations. The role emphasizes data-driven decision-making, collaboration with health partners, and ongoing professional development for staff.

Qualifications

• Licensed healthcare professional in good standing with the applicable college.
• Bachelor’s degree in healthcare management, business administration or related field is an asset.
• Minimum 5 years in a leadership role within healthcare/home care.
• Knowledge of health care legislation (e.g., Mental Health Act, Privacy Act).
• Strong leadership, communication and analytical skills.

Responsibilities

• Lead clinical supervisors and front-line staff to ensure care delivery quality.
• Coach and mentor staff, oversee care and service delivery performance.
• Monitor clinical indicators and ensure mandatory training completion.
• Manage on-call schedules and respond to clinical concerns.

What You’ll Need

• Regulated health care professionals in good standing with the applicable College in the Allied Health field.
• Bachelor's degree in healthcare management, business administration, or a related field is considered an asset
• Minimum of 5 years of experience in a leadership role within the healthcare industry and/or home and
• Working knowledge of applicable health care legislation including Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act and others
• Effective skills in critical thinking and problem solving
• Flexibility in hours worked in order to accommodate operational needs
• Knowledge of applicable community resources available for clients and families
• Current certification (within the past 12 months) of First Aid, CPR, completed within Canada
• Expertise in data-driven research and analysis, with the ability to translate insights into actionable
• strategies and decisions.
• Exceptional leadership abilities, with a demonstrated ability to guide, support and motivate teams to achieve their full potential.
• Excellent communication and interpersonal skills, with the ability to effectively engage and collaborate
• with stakeholders at all levels of the organization.
• Compassionate and empathetic leadership style, with a genuine commitment to providing the highest level of care and support to the team, clients and patients.
• A current registration in a regulated professional college and professional association is an advantage.
• Excellent computer skills including experience in Word and Excel Able to provide a current, clear criminal back check with Vulnerable Position Screening (VPS)

What You’ll Do

• Show respect for cultural needs, dignity, privacy and confidentiality of staff and clients
• Coach and mentor clinical supervisors on their role in overseeing care delivery and on how to support front line team members and patients and their families
• Monitor care and service delivery performance and clinical indicators
• Participate in the interviewing, orientation, ongoing supervision, and performance management of clinical supervisors
• Ensure completion of all mandatory training and skill development
• Create and maintain the on-call schedule for Clinical Supervisors
• Respond to clinical concerns from staff, contractors, funders, clients and families
• Review all client safety incidents and participate in the investigation and resolution
• Provide clinical leadership at the district
• Provide oversight of CQI activities related to care and service including staff participation and problem-solving teams
• Ensure compliance to ParaMed policies, procedures and legal requirements for yourself and all staff
• Participates in necessary care conferences with Health Care team, Contractors and funders
• Work collaboratively with a multi-disciplinary team consisting of government agencies, healthcare workers, and other community resources
• Demonstrate continuous learning and professional development through involvement in conferences, committees and networks
