Job summary

AgeCare in Lumby, British Columbia is seeking a Social Worker to support residents and families with assessments and move-in processes in a long-term care home.

The role includes coordinating with the Director of Care and ensuring access to community resources, with a focus on maintaining residents' health and well-being. At least a Bachelor’s or Master’s in social work and 1 year of health care experience are preferred.

The Opportunity:

The Social Worker assists residents and their families by providing support and assessment for residents, related but not limited to, move-in process, transitions, finances, concerns and decline in health as well as assistance accessing community resources.

What Is In It For You:

• Assists the Director of Care and Business Manager with the process of moving new residents into the home by coordinating with incoming residents and their Power of Attorney (POA) and providing support with aspects of the move-in process that require the involvement of the Social Service Worker.
• Participates in the residence’s Continuous Quality Improvement (CQI) program.
• Evaluates the effectiveness of Social Service Work services on an annual basis and provides input to update policies and procedures that impact the Social Service Work practice.
• Prepares and submits reports to the residence and/or corporate office as required.

What You Will Need:

• Bachelors or Master’s Degree in social work.
• Minimum one-year relevant experience preferably in a health care setting.
• Previous experience in a long-term care setting is preferred.

Salary Range: $42.27 - $52.81 per hour

Salary placement based on qualifications and experience.
