Job summary

AgeCare is seeking a Casual Practical Nurse in Lumby, BC to join the inter-professional team and deliver person-centered care. You will conduct resident assessments, develop and update care/service plans, and document care in adherence to regulatory standards.

The role requires a Practical Nurse Diploma, current registration, CPR, and 3+ years of seniors’ health experience, with proficiency in Microsoft Office. A supportive culture and meaningful work await in this community setting.

Qualifications

• Must hold Practical Nurse Diploma and be registered in good standing in the applicable province.
• Minimum of 3+ years of experience with seniors’ health.
• Current CPR certification.

Responsibilities

• Work as a member of the inter-professional team applying the person-centered care model.
• Complete resident clinical assessments, care/service plans and documentation.
• Implement and evaluate care/service plans and communicate changes as required.
• Assist with scheduling of resident care conferences and participate in them.
• Collaborate with the Director of Care to facilitate move-ins, moves and transfers.
• Provide direct resident care and medications/treatments with documentation as per standards.

What You Will Do

• Work as a member of the inter-professional team as described by our Professional Practice Model applying the person-centered approach to care.
• Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.
• Implement and evaluate the effectiveness of the care/service plans and make changes as required.
• Assist with scheduling of resident's care conferences and participate in conferences where applicable.
• Work closely with the Director of Care to facilitate resident's move in, move out and transfers.
• Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards and policies and procedures.

What You Bring

• Practical Nurse Diploma.
• Current registration and membership in good standing in applicable Province.
• 3+ years of experience with seniors’ health.
• Current CPR training.
• Computer proficiency and experience with Microsoft office applications.
• Demonstrates leadership, coaching and mentorship skills.

Wage Levelling Salary: $32.84 - $41.35 per hour Salary placement based on qualifications and experience.
