Schedule
The successful candidate will be available to work Monday through Sunday in 8-hour shifts, primarily between 08:30 and 16:30. Casual staff must provide reasonable availability, including at least two weekdays and two weekends within each 6-week period. The candidate will also rotate to cover the Uxbridge site remotely. Schedule is subject to change based on operational needs.
Compensation
Salary Grid : OPCOM13 : $ - $ per hour.
Responsibilities
Patient care
As an integral member of the inter-professional team, you will provide social work services to patients and their families within the organization, primarily in the acute care setting, excluding inpatient mental health. Emphasis is on psychosocial support, interdisciplinary problem solving, and liaising with team members to connect patients and families with necessary resources and supports.
Leadership
You will demonstrate exceptional leadership, professional integrity, and accountability. You will play a key role in system development and possess a strong understanding of care across the continuum. Experience in short-term supportive counseling, especially with patients and families facing difficult decisions or end-of-life issues, is valuable.
Support
Responsibilities include completing psychosocial assessments, providing short-term goal-oriented support and counseling for grief, chronic issues, and end-of-life concerns, developing patient care plans with the healthcare team, cross-coverage for other social workers, and consulting with community healthcare professionals.
Consulting
Engage in consultation with community health care professionals to coordinate care and resources.
Community Resources
Maintain up-to-date knowledge of community resources to support patients and families effectively.
Requirements
Education
Master of Social Work and registration with the Ontario College of Certified Social Workers and Social Service Workers.
Experience
Recent social work experience in a hospital setting, with experience using electronic documentation systems such as Meditech.
Skills
Understanding of loss, adjustment, and grief processes; interpersonal, problem-solving, and conflict resolution skills; excellent communication; commitment to professional development.
Initiative
Good attendance and performance records; commitment to a safe and healthy workplace, compliance with policies, legislation, and mandatory education.
