Company Description

Our Client, who was founded in 2017, is a Canadian healthcare provider specializing in integrative medicine through a virtual medical clinic. With a focus on assisting Veterans of the Canadian Armed Forces, First Responders (Paramedics, Police, RCMP, and Firefighters), frontline workers, as well as the general public, the company aims to reduce dependencies on opioids by directing patients towards alternative treatment, like medical cannabis.

On top of medical cannabis prescriptions, the company also offers primary treatment prescriptions, as well as topical pain solutions, psycho-social care, dietician and naturopathic services, and overall patient support. The clinic is fully virtual, and has a team of approximately 15-20 employees throughout Canada.

Role Description

The Social Worker plays a critical role in supporting the mental health and well-being of patients by providing assessment, counselling, and case management services. This role focuses on assisting individuals, particularly Veterans and First Responders, in navigating healthcare and social support systems. The ideal candidate is compassionate, resourceful, and experienced in working with diverse patient populations in a virtual healthcare setting.

This role is essential to helping our client broaden their patient support to Quebec. They are looking for a candidate with an initial availability of 2 to 4 hours per week, flexible around your schedule, with the potential for additional hours as the patient base expands.

Responsibilities :

  • Conduct psychosocial assessments and develop individualized treatment plans.
  • Provide virtual counselling and therapeutic interventions to patients.
  • Coordinate care by liaising with healthcare providers, support organizations, and government agencies.
  • Assist patients in accessing mental health, financial, and community resources.
  • Ensure compliance with healthcare policies, privacy laws, and ethical guidelines.
  • Maintain accurate records and documentation in accordance with regulatory standards.
  • Support crisis intervention and suicide prevention efforts as needed.
  • Participate in professional development and continuing education to stay updated with best practices in social work.
  • Work closely with nurse practitioners, and other healthcare professionals to ensure holistic patient care.

Preferred Qualifications and Skills :

  • Master’s degree in social work (MSW) from an accredited institution.
  • Licensed / Registered Social Worker in Quebec – a registration in multiple provinces is an asset.
  • Experience working with Veterans, First Responders, or individuals facing trauma-related conditions is preferred.
  • Successful completion of a vulnerable sector check.
  • Strong interpersonal and communication skills, with the ability to establish trust with patients.
  • Knowledge of mental health conditions, trauma-informed care, and evidence-based therapeutic approaches.
  • Proficiency in digital health platforms and virtual communication tools.
  • Ability to work independently while maintaining strong collaboration within a remote team.
  • Experience in case management and patient advocacy.
  • Familiarity with Veterans Affairs Canada (VAC) and other social support programs is an asset.
  • Fluency in French is a must.
