Job summary

University Health Network (UHN) in Toronto, Ontario, Canada, is seeking a temporary full-time registered nurse for the Toronto Rehab Institute site. The role supports Complex Continuing Care & Dialysis in the South 3 unit, reporting to the Manager, Program Services.

Qualifications

• Current registration with the College of Nurses of Ontario (CNO)
• Basic Cardiac Life Support (BCLS) certification is required
• Ability to work effectively within an interdisciplinary patient care team
• Must meet the physical requirements of the position
• Excellent interpersonal, communication and decision-making skills
• Excellent organizational and time management skills
• Excellent computer skills
• BScN degree preferred (optional)

Responsibilities

• Plan, manage, evaluate and coordinate comprehensive professional nursing care for patients and families within the context of an interdisciplinary practice
• Use theory and input from team members to plan and implement care for Complex Continuing Care patients and families
• Educate patients, families, referring facilities and community providers about dementia care
• Participate in inter-professional team care planning forums, rounds and family conferences
• Provide leadership, supervision and mentorship to students and new staff on the unit
• Engage in professional development and hospital-wide initiatives
• Contribute to patient safety through adherence to hospital plans and policies

Duties

• Plan, manage, evaluate and coordinate comprehensive professional nursing care for patients and families within the context of an interdisciplinary practice
• Use theoretical and empirical knowledge and the input of team members to plan and implement care for Complex Continuing Care patients and families
• Education of patients, family, referring facilities and other community providers regarding dementia care
• Participate in inter-professional team care planning forums, inter-professional rounds and family conferences
• Provides leadership, supervision and mentorship to students and new staff on the unit related to Complex Continuing Care
• Participate in professional, program and hospital-wide initiatives, including professional development of self
• Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients
