Job summary

University Health Network (UHN) in Toronto is seeking a Community Health Worker - Vocational Specialist to support individuals with serious mental illness in pursuing employment, education, volunteer opportunities and meaningful community roles aligned with recovery goals.

As a member of the IMPACT interdisciplinary team, you will provide individualized vocational support, facilitate group programming, and collaborate with community partners to promote social inclusion and skill development.

Qualifications

• Completion of a Community Health Worker, Social Service Worker, or related diploma program, or equivalent combination of education and experience.
• 1–2 years of experience in supporting individuals with mental illness, substance use, homelessness, or complex social needs.
• Knowledge of recovery-oriented practice, harm reduction, trauma-informed care, and anti-oppressive approaches.
• Experience facilitating groups, workshops, or psychoeducational programming.
• Excellent interpersonal, communication, engagement, and problem-solving skills.

• Minimum one (1) to two (2) years' experience supporting individuals experiencing mental illness, substance use, homelessness, or complex social needs.
• Demonstrated knowledge of recovery-oriented practice, harm reduction, trauma-informed care, and anti-oppressive approaches.
• Experience facilitating groups, workshops, or psychoeducational programming.
• Excellent interpersonal, communication, engagement, and problem-solving skills.
• Ability to work independently in community environments and collaboratively within a multidisciplinary team.
• Demonstrated computer proficiency and ability to document electronically.
