Job summary

An established industry player is seeking a dedicated Social Worker to join their team in London, Ontario. This full-time position involves engaging with residents and their families, facilitating a resident-oriented environment, and providing essential social work services. You will conduct pre-admission screenings, promote advocacy, and lead interdisciplinary team care conferences. Job description

Home: Southbridge London

Location: London, Ontario

Position: Social Worker - Full Time

Key Responsibilities
  1. Receive and respond appropriately to inquiries pertaining to Southbridge London which relate to new resident placement, including conducting tours of the home with prospective residents.
  2. Conduct pre-admission procedures including: Screening of applications; Correspondence with applicants to acknowledge receipt of application and to provide information; Pre-admission visits and assessments; Communication with placement agencies; Navigates the resident/family through the admission process including coordination with the LHIN.
  3. Promote and facilitate a Resident-oriented environment, acting as a Resident/family advocate. Coordinates and is the home's liaison person for Family Council. Leads the Customer Service program within the home.
  4. Coordinate, plan and participate in annual and new admission interdisciplinary team care conferences for residents.
  5. Plan, organize and carry out social work programs/services to meet the ongoing psychosocial needs of the Residents and their families/advocates.
  6. Provide palliative care support to residents and family as needed.
  7. Examine and act on matters concerning possible financial abuse or emotional abuse of a resident and work collaboratively with the interdisciplinary team to address the issue.
  8. Lead the ethics committee for the home and be available to staff for consultation regarding ethical dilemmas within the home, working collaboratively with staff to resolve ethical dilemmas.
  9. Participate on committees related to resident care and operation of the facility as required.
  10. Function as a member of the multi-disciplinary team in planning overall Resident care.
  11. Provide educational and consultative services on social work related issues/topics through participation in staff orientation programs, staff development programs and educational seminars for staff, Residents and families/advocates as required.
Key Qualifications
  1. Registered under the Social Work & Social Service Act, 1998; Graduate of a recognized school of social work with a Bachelor of Social Work degree.
  2. Experience in working with Geriatrics; Familiarity with the multi-disciplinary team concept, skill in teamwork and ability to work effectively in a multi-disciplinary setting.
  3. Minimum of 3 years' experience as a Social Worker in a clinical setting.
  4. Strong knowledge base for mental health issues.
  5. Excellent team player with strong interpersonal and communication skills.
  6. Ability to respond to crisis situations with demonstrated critical thinking.
  7. Demonstrated ability to function as a team player.
  8. Good communication and interpersonal skills.
  9. French Language skills will be considered an asset.
