Job description

Alexandra Hospital, Ingersoll and Tillsonburg District Memorial Hospital are actively seeking applicants for an Integrated Discharge Planner.

Key Responsibilities

  • Complies with personnel policies and hospital safety policies.
  • Maintains confidentiality when interacting with patients, families, personnel, and the public.
  • Maintains compliance with professional practice guidelines and standards.
  • Performs duties as prescribed by the discharge plan of the hospital and as directed by the clinical manager.
  • To promote discharge planning services to facilitate a timely discharge to a safe and healthy environment.
  • Provides case management, social services, utilization review, and discharge planning daily or as scheduled or assigned by the clinical manager.
  • Reviews information on patients in assigned areas necessary to facilitate discharges in a timely manner.
  • Participates in continuing education relevant to the field of expertise of current practice and other pertinent and appropriate learning experiences to maintain and increase personal and professional growth.
  • Maintains clinical competency in discharge planning/case management.
  • Begins initial discharge planning assessment within 24 hours of admission.
  • Using scoring from initial assessment, determines the degree of discharge planning risk on an individual basis for each patient.
  • Maintains required reports, records, and statistical information in the electronic medical record.
  • Ensures coordination among all departments within the hospital in furthering the mission of the facility.
  • Knows and follows infection prevention and control (IPAC) guidelines.

Qualifications Required

  • Regulated healthcare worker in good standing with the appropriate college or regulatory body.
  • Social work preferred: preference will be given to candidates with a bachelor's degree in social work. A master's degree in social work is preferred.
  • Minimum of five years of experience in an acute care hospital setting.
  • Good verbal and written communication skills to interact effectively with patients, families, interdisciplinary teams, and community stakeholders.
  • Knowledge of available community resources and services to support patients' post-hospitalization needs.
  • Previous experience in discharge planning, including coordinating post-discharge care and services.
  • Experience working with individuals with mental illness and addressing their unique needs.
  • Ability to contribute to the development, implementation, and evaluation of discharge planning programs and initiatives.
  • Strong analytical and critical thinking skills to assess patient needs and develop appropriate discharge plans.
  • Demonstrated ability to identify and address complex patient care issues and challenges.
  • Compassionate and empathetic approach towards patients and their families during the discharge planning process.
  • Awareness and sensitivity to diverse cultural backgrounds and their impact on healthcare decisions and preferences.
  • Compliance and commitment to adhering to regulatory requirements, hospital policies, and professional standards in all aspects of discharge planning.

Other Pertinent Conditions:

  • Salary as per the Non-Union Salary Schedule ($47.09 per hour to $55.09 per hour)

Must be available to work all shifts as required, which may include; weekends, statutory holidays, days, evenings and night shifts. Hours are subject to change based on operation needs of the hospital.
