SE Health is seeking a Social Worker to deliver person-centered care in clients' homes and communities. You will apply clinical expertise to help clients achieve goals and maximize independence, building meaningful therapeutic relationships along the way.

You will join a diverse, collaborative team with strong leadership, and enjoy a hybrid work model with ongoing education, mentorship, and growth opportunities across Canada.

Qualifications

  • Graduate of an accredited Social Work program and in good standing with the Ontario College of Social Workers and Social Service Workers.
  • Experience in acute/rehab or community-based rehabilitation programs preferred.
  • Ability to work independently and collaboratively within an interdisciplinary rehabilitation team to support positive client outcomes.

Requirements
  • A graduate of an accredited program in Social Work and are a member in good standing of Ontario college of Social Workers and Social Service Workers
  • Recent experience in managing patients inacute/rehab or community-based rehabilitation programs is preferred
  • Demonstrated ability to work independently and collaboratively within an interdisciplinary rehabilitation team while supporting positive client outcomes.
  • Sound knowledge of community resources is an asset
  • Effective interpersonal, organizational and communication skills
  • A positive work and attendance record is essential
  • Ability to adapt to new computer applications
  • This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community-based rehabilitation setting
