The Social Service Worker will help support patients to manage personal and social challenges, offering short-term counselling, facilitating group programming and connecting individuals with community resources. In collaboration with the Interprofessional Healthcare Team, the Social Service Worker participates in the assessment, treatment and evaluation of individual, interpersonal and societal problems using a social service framework.
