AMHS-KFLA is seeking a Social Worker in Kingston for the NSACTT program. The role involves independent treatment, rehabilitation and support to assist community living and recovery for clients with serious mental illness.

Job Summary: The Social Worker works as a member of an interdisciplinary team independently carrying out treatment, rehabilitation and support functions to facilitate community living, psychosocial rehabilitation, and recovery within support functions to facilitate community living, psychosocial rehabilitation and recovery for persons living with a serious mental illness. This incumbent is responsible for coordinating the individualized treatment plan and required services for assigned clients. Services are provided in the environment of choice of the individual wherever possible.
In addition, the incumbent participates in the comprehensive assessment of clients and shares in the responsibility for addressing the needs of all clients requiring frequent contact. Social work expertise is shared with other team members to assist in clients’ engagement and partnership with family members; entitlements; and advocacy. All services are provided in accordance with the mission, vision, values and established protocols of the Agency, and the principles of psychosocial rehabilitation.

Social Worker, Assertive Community Treatment Team (ACTT) works within the ACT team as a Mental Health professional Social Worker carrying out treatment, rehabilitation and support functions to facilitate community living, psychosocial rehabilitation, and recovering within the contact of the ACT model for persons living with a serious illness.
