Work Schedule: Monday to Friday with ability to work evenings and weekends as needed

Salary: Starting at $44.00/hour

Harbourview Family Health Team (HFHT) is a well-established primary care team based in Thunder Bay, dedicated to delivering high-quality, team-based care to our patients and their families. Our interdisciplinary team includes nurse practitioners (NPs), registered nurses, registered dietitians, social workers, administrative professionals, and 14 family physicians.

Job Summary:
We are seeking qualified applicants for our core care teams which provide an integrated, team-based approach to care with a team consisting of physicians, nurse practitioner, social worker and a registered practical nurse. These teams are designed to enable patients to receive the right care at the right time. The core teams work with a specific patient panel, providing better access to and continuity of care.
The social worker is an integrated member of a core team, working to their full scope of practice to support the management of the pod's collective patient panel. The social worker focuses on mental health assessments, case management, care coordination, and referrals to community resources. The social workers support patients by delivering psychoeducation to promote wellness, self-management, coping skills, recovery, crisis intervention, and support for mild-to-moderate mental health challenges. The social workers are an integral part of our service delivery, but are also an asset to our team and organizational culture.
The social worker practices independently within their professional scope and competence as regulated by the Ontario College of Social Workers and Social Service Workers.
