Job summary

The University Health Network in Toronto is seeking a Registered Nurse for a 0.5 FTE position in the 2C - Stroke/Neuro unit at West Park Healthcare Centre.

You will plan, manage and evaluate comprehensive nursing care, collaborate with the inter-professional team, educate patients and families, and contribute to program and hospital-wide initiatives. UHN supports ongoing professional development and a flexible, collaborative work environment.

Qualifications

• Diploma in Nursing required; Baccalaureate preferred.
• Active registration with College of Nurses of Ontario.
• Neuroscience nursing experience appreciated.
• Ability to perform essential nursing duties.

Responsibilities

• Plan, manage, evaluate and coordinate comprehensive professional nursing care.
• Provide care for stroke/neuro patients within an inter-professional team.
• Educate patients, families and others on stroke/neuro care.
• Engage in inter-professional rounds and care planning discussions.
• Supervise and mentor students and new staff.
• Participate in hospital-wide initiatives and professional development.

Wage Range: $41.15 - $58.98 per hour (start rate in accordance with collective agreement)

Hours: 0.5 FTE

Shift: Days/Nights

Status: Permanent Part Time

Position Summary

The role of the Registered Nurse is the promotion of health and the assessment of, the provision of care for and the treatment of health conditions by supportive, preventative, therapeutic, palliative and rehabilitative means in order to attain or maintain optimal function.

Duties

• Plan, manage, evaluate and coordinate comprehensive professional nursing care for patients and families within the context of an inter-professional practice and primary nursing model
• Use theoretical and empirical knowledge and the input of team members to plan and implement care for patients with stroke/neuro and their families
• Education of patients, family, referring facilities and other community providers regarding care of patients with Stroke/Neuro
• Participate in inter-professional team care planning forums, inter-professional rounds and family conferences
• Provide leadership, supervision and mentorship to students and new staff on the unit related to Stroke/Neuro Rehabilitation nursing
• Participate in professional, program and hospital-wide initiatives, including professional development of self
• Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients.

Qualifications

• Diploma in Nursing
• Baccalaureate degree in Nursing from an approved nursing program preferred
• Member in good standing with the College of Nurses of Ontario
• Neuroscience nursing experience preferred
• Ability to perform the essential clinical duties of the job
• Ability leading others, including the effective management of challenging interpersonal situations
• Effective communication, problem-solving and decision-making and reliable work habits
• Commitment to continuing professional development
• Demonstrated commitment to team success
• Resilience and effective coping strategies to manage change
• Computer literacy in word processing and Electronic Patient Record
• Working knowledge of the College of Nurses of Ontario standards of practice
• Able to perform multiple admissions assessments in EPR on a daily basis.
• Able to work cohesively with the interdisciplinary team in a fast pace environment
