Job summary

Neighborhood Healthcare PACE seeks a Referrals Specialist to coordinate referrals and follow‑up care for our participants. You will receive, process, schedule, and track referral requests for diagnostic testing and specialty care, working onsite in a clinical setting.

This is a full-time temporary role lasting about 3 months, with a Monday–Friday, 8am–5pm schedule. You will liaise with providers, manage transportation, and maintain EMR records to ensure timely authorizations and high‑quality

Qualifications

• High school diploma or GED required.
• One year of clinical or healthcare experience required.
• Experience with referral authorization and data processing preferred.
• Bilingual English/Spanish highly preferred.
• Current Basic Life Support (BLS) certification required upon hire; CPR and First Aid certification required.

Responsibilities

• Coordinate referrals and scheduling for PACE participants.
• Process and complete referrals accurately and timely.
• Arrange transportation for participants to medical appointments.
• Communicate referral details to participants and families.
• Track referrals in EMR and follow up on authorizations.
• Escalate issues until the referral loop is closed.
• Assist with surgery scheduling and follow ups; include orders and tests.
• Document actions in the medical record per regulations.
• Act as liaison between participants, providers and resources.
• Answer referral inquiries via phone, portal, or mail.
