We’re Hiring a Rural Youth Outreach Case Manager!

Are you passionate about making a difference in the lives of rural youth? Cornerstone Landing Youth Services is seeking a dedicated Rural Youth Outreach Case Manager to join our team and help break down barriers to housing for youth in Lanark County.

As a Rural Youth Outreach Case Manager, you’ll play a vital role in empowering at-risk youth by providing mobile, personalized outreach and case management. You’ll meet youth where they are—at home, school, or in the community—helping them access stable housing, mental health support, and essential wraparound services. This is your chance to be a key advocate and resource for rural youth, working both independently and collaboratively with local partners.

Qualifications:

Diploma or Degree in Social Work, Child & Youth Care, Psychology, or a related Human Services field.

2+ years of experience working with at-risk or homeless youth.

Strong relationship-building, communication, and organizational skills.

Critical thinking, problem-solving, and adaptability.

Sensitivity to diverse backgrounds and lived experiences.

Valid Ontario driver’s licence and access to a vehicle.

Key Responsibilities:

Engage youth through mobile outreach in homes, schools, and community spaces.

Conduct needs assessments and develop individualized service plans.

Support youth with housing searches, applications, and tenancy.

Connect youth to employment, life skills, and mental health resources.

Provide on-site support at our transitional house and facilitate group activities.
