Qualifications

• Current registration with the College of Nurses of Ontario.
• CPR certificate is required.

• Willingness to attain Saint Elizabeth certifications.

Responsibilities

• Perform comprehensive client assessments and develop care plans based on best practices.
• Serve as primary provider for a client caseload when assigned, coordinating with care team.
• Support transitions by connecting clients with community services.
