Qualifications

• Current registration with the College of Nurses of Ontario (CNO) is required.
• CPR certificate is required.
• A valid driver’s license and own vehicle, travel to client sites, are required.
• Updated immunization records and background checks (criminal and Vulnerable Sector) are required on hire.
• Willingness to attain SE Health certifications and workdays/evenings/nights including every other weekend.

Responsibilities

• Perform comprehensive client assessments and develop/evaluate care plans.
• Serve as primary provider for a client caseload and coordinate with the care team.
• Support transitions by connecting clients with community services.
