Job summary

SE Health is hiring a part-time Registered Practical Nurse (RPN) to work in the Women's College Clinic in Downtown Toronto. The role offers flexible schedules, paid training, a $2,000 sign-on bonus, and competitive benefits with a supportive team.

As primary provider for a client caseload, you will complete assessments, develop and evaluate care plans, and coordinate with the care team while meeting credential and immunization requirements.

Qualifications

• Current registration with the College of Nurses of Ontario
• Canadian Nurses Association (CNA) specialty certification an asset
• Enthusiasm and love of community care and evidence-informed practice
• A current CPR certificate is required
• A valid driver’s license and vehicle (travel required)
• Updated immunization records, clear criminal background check, and Vulnerable sector check on hire
• Willingness to attain Saint Elizabeth certifications
• Availability to work days, evenings and every other weekend
• Physically demanding position with lifting and movement requirements

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, be the point of care lead and collaborate with the care team
• Support the client through care transitions by connecting them with community support services
