Qualifications

• Current registration with the College of Nurses of Ontario.
• CPR certification is required; CNA certification is an asset.

Responsibilities

• Complete comprehensive client assessments and develop care plans.
• Lead care for designated client caseload in collaboration with the team.
• Assist clients through transitions by connecting with local support services.
