Renfrew Victoria Hospital is launching ALIGN, a program focused on safe hospital-to-home transitions, with a full-time Registered Practical Nurse (RPN) role. The RPN will work across inpatient and community settings, providing assessments, medications, education, and discharge planning.

ALIGN emphasizes coordinated, patient-centred care, collaboration with multidisciplinary teams, and support for complex needs to reduce delays and improve outcomes for patients and families.

Qualifications

• Must hold current registration with the College of Nurses of Ontario (CNO).
• BCLS certification required.
• Experience in medical, surgical, geriatric, or community nursing preferred.
• Ability to work independently and make sound clinical decisions.
• Strong communication, assessment, and care coordination skills.
• Excellent organizational and time-management abilities.
• Valid driver’s license and reliable transportation.

Responsibilities

• Work as part of the inpatient nursing team and perform routine nursing duties.
• Conduct clinical assessments and administer medications and treatments.
• Monitor patient status and provide discharge education.
• Support transitions from hospital to home and coordinate care across settings.
