Zusammenfassung

CarePartners is hiring a Personal Support Manager to coordinate and supervise Community Health Services across Tillsonburg and St. Thomas regions. The role involves leading the Community Support Workers, developing Care Plans, and ensuring compliance with health regulations.

The ideal candidate is an RN or RPN with 3–5 years of relevant leadership experience, CCS registration, and strong communication skills in English; CPR and a background check are required.

Qualifikationen

• RN or RPN registered with a nursing college (CNO)
• 3–5 years of relevant job experience
• Registration with the College of Nurses of Ontario (CNO)
• Experience in community healthcare is an asset
• Leadership experience is an asset
• Proven computer skills (email, word processing, scheduling)
• Valid driver’s license; ability to travel as required
• English proficiency; French is an asset
• CPR certification
• Clear Vulnerable Sector Check

Aufgaben

• Provides coordination, supervision, and coaching to Community Support Workers
• Develops and maintains an effective Care Plan with respect to Community Health Services
• Keeps abreast of policies in the Regulated Health Professions Act and CHS best practices
• Promotes and markets Community Health Services and maintains positive public relations
• Provides active guidance for workplace Health and Safety
• Performs other duties as required
