Job summary

CarePartners is seeking a Personal Support Manager to coordinate and supervise Community Health Services across Ontario, with travel to Elliot Lake. This full-time role includes on-site work with on-call rotations in the evenings and weekends.

The position requires an RN or RPN and CNO registration, 3–5 years of leadership in community health, strong English communication, and reliable computer skills. A vehicle and CPR certification are assets offered within a supportive team.

Qualifications

• RN or RPN degree/diploma and active CNO registration.
• 3–5 years of relevant experience in community health.
• Strong English communication and computer skills; vehicle and travel required.

Responsibilities

• Coordinate, supervise, and coach Community Support Workers in the field.
• Develop and maintain Care Plans with respect to Community Health Services.
• Stay updated on Regulated Health Professions Act policies and best practices.
• Promote and market Community Health Services and maintain positive public relations.
• Provide active guidance for workplace Health and Safety.
• Perform other duties as required.
