Job summary

SE Health is hiring Registered Nurses in Ontario for a per-visit home care role. You will assess clients, plan and evaluate care, and lead a client caseload in partnership with the care team. Travel is required to meet clients in the community.

Qualifications

• Current registration with the College of Nurses of Ontario.
• CNA specialty certification an asset.
• CPR certificate required.
• Valid driver’s license and vehicle (travel required).
• Immunization records, background check and Vulnerable sector check required on hire.
• Willingness to attain Saint Elizabeth certifications.
• Availability to workdays and every other weekend.
• Physically demanding role with lifting up to 55 pounds.

Responsibilities

• Complete comprehensive client assessments and develop care plans.
• Lead care as primary provider for a client caseload in collaboration with the team.
• Support clients through transitions and connect with community services.
