Job summary

SE Health is seeking full-time nurses to cover Roseneath and surrounding areas in Ontario. The role offers a per-visit pay model with guaranteed visits and a focus on community care within a national social enterprise.

Qualifications

• Current registration with the College of Nurses of Ontario.
• CNA specialty certification an asset.
• Enthusiasm and love of community care and passion for evidence-informed practice.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and client’s needs.
• When designated as a primary provider for a client caseload, be the point of care lead and collaborate with the care team.
• Support the client through care transitions by connecting with community support services.
