Job summary

SE Health is seeking registered nurses to deliver high-quality care in the community setting. You will perform comprehensive assessments, plan and evaluate care based on best practices, and collaborate with the care team to meet client needs.

You will serve as the primary provider for assigned caseloads, coordinating transitions and connecting clients with community resources. A valid Ontario RN registration, CPR, and ability to travel are required.

Qualifications

• Current registration with the College of Nurses of Ontario.
• CPR certificate required; immunization records and background checks.
• Willingness to attain Saint Elizabeth certifications and travel.

Responsibilities

• Perform comprehensive client assessments and develop care plans.
• Act as primary provider for a client caseload and lead care delivery with the team.
• Support client transitions by linking to community services.
