Job summary

SE Health is seeking a registered nurse or registered practical nurse to support clients across Ontario. The role offers a caring team, flexible schedules including days/evenings/nights, and opportunities to grow within a national social enterprise.

You’ll complete assessments, develop and implement care plans, and collaborate with the care team and community services. A Certificate of Competence with CNO, CPR, immunizations, and background checks are required.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend for 12 hour shifts.
• Updated immunization records, clear criminal background check, and Vulnerable sector check.
• Willingness to attain SE Health certifications.
• A current CPR certificate is required.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
