Placements24 is seeking a Social Support Facilitator to join our remote team serving communities nationwide. You will coordinate and deliver essential social support services through virtual platforms, manage client interactions, and guide access to benefits and services.

This role emphasizes independence, empathy, and strong organizational skills as you collaborate with remote colleagues and partners to ensure accessible support and efficient service delivery.

Qualifications

• Bachelor's degree in Social Work, Psychology, or a related field.
• Proven experience in social work, case management, or client support services.
• Strong proficiency with virtual communication and collaboration tools (e.g., Zoom, Teams).
• Excellent organizational skills and attention to detail for managing electronic records.
• Ability to work independently and manage a caseload in a remote setting.
• Demonstrated empathy and strong interpersonal skills for effective virtual client engagement.

Responsibilities

• Facilitate virtual client intake, assessment, and ongoing support sessions.
• Coordinate and connect clients with appropriate community resources and social services remotely.
• Develop and manage digital tools and platforms to enhance service delivery and client engagement.
• Provide information and guidance on accessing benefits, support programs, and essential services.
