Job summary

McKesson is seeking a compassionate Part‑Time Nurse Case Manager to support patients, healthcare providers, and clinic partners through a specialized Patient Support Program. You will educate patients, coordinate care, and facilitate access to treatment, working roughly 2.5 days per week with hours 8:00 a.m. to 8:00 p.m.

EST. This primarily remote role requires licensing in Alberta or British Columbia and travel to clinics across AB/BC.

Qualifications

• Active nursing license in Alberta and/or British Columbia.
• Diploma/degree or equivalent with P1 Career Framework.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office and electronic documentation systems.
• Ability to travel to clinics across Alberta and British Columbia.
• Ability to work ~2.5 days per week within operating hours.

Responsibilities

• Primary contact for patients in the Patient Support Program.
• Educate on prescribed therapies, administration, and adherence.
• Support patient enrollment and onboarding.
• Coordinate care with physicians, clinics, pharmacies, and healthcare professionals.
• Assist patients in navigating access to treatment and reimbursement.
• Conduct patient assessments and follow-ups as required.
• Monitor progress and provide ongoing support.
• Document accurately in program systems.
• Identify and escalate clinical or operational concerns.
• Contribute to quality improvement initiatives and program success.
• Build and maintain relationships with healthcare providers.
